German frigate had a missile misfire on the 21st.

Comments mention only two people being injured, probably from the bridge and from hurting themselves when it went off. Looks like maybe the SM-2 wasn't released and cooked off in the canister causing an explosion?

Edit - Actually looking at the aftermath pictures, looks like the cell door didn't open up all the way.

That seems to have left a mark.

 I'm wondering if the door may have blown back partially closed from the force of the explosion? I'd think that had the thing gone off completely in the launch tube that the surrounding tubes would have shown a lot more damage. Just a guess, though. I know those things are specifically hardened to prevent a chain fire/explosion in events exactly like this.

There's the problem.  They didn't say "please" when they asked HAL to open the missile bay doors.

Yeah, but you'd think there would be a little more shielding between the tubes and the bridge windows just in case of something like this.

The bridge windows didn't even lose their glass.  Why would you add more shielding?
