It was apparent to me that he was suffering from some form of dementia during the primary campaign.  Whatever form he has, has accelerated since the time he took office.
Alzheimer's killed my maternal grandmother and my mother.  It's killing my father-in-law right now.

Alzheimer's with my great great grandmother, generalized dementia with my dad near the end.  Biden is somewhere less an grandma who was having full on hallucinations, but far worse than dad who still had lucid periods; biden doesn't seem in any way lucid, not even for brief moments.

If you watch complete, unedited videos of him speaking at news conferences, interviews, etc., it's obvious that he has problems.  It becomes very apparent if you compare recent video with content shot years ago before his decline began.

Rumor has it (and it's just rumor, nothing more) he was thoroughly rehearsed ahead of time on the questions that were going to be asked, and then pumped full of drugs that temporarily mitigated his memory issues.
I remember watching excerpts of some of his performances where Biden went off track a bit during the debates.  Those episodes weren't anywhere near as bad as what we are seeing now.  His impairment has gotten significantly worse over the last ten months in my opinion.

The media waged a pretty impressive psyop on the public.

They just ignore the small percentage of time Bidens gafs make up, and have set the bar so low that if Joe gets up there and just makes a speech at the 10th grade level he is good to go as President.
