An apparent explosion at a chemical plant in Texas late Tuesday has killed two people and injured at least seven in what local authorities described as a “mass-casualty incident.”

The leak of acetic acid, a food preservative that is commonly used in vinegar and can be flammable, happened at 7:35 p.m. Tuesday at the LyondellBasell facility, which is the world’s third-largest producer of the acid, according to the company.

I heard about it this morning.  Not sure what exactly happened yet, but my first guess is something got opened up that wasn't locked out properly.  Maybe not.  We will see.

Fair point.  I wouldn't rule out a crane dropping something.
